A monitoring platform continues to generate alerts as expected. Dashboards look stable. However, a subset of transactions never reached the platform due to a silent upstream ingestion issue. No alert can ever be generated for data that never arrived.
Correct volume, incorrect meaning
Record counts reconcile perfectly across systems. Completeness appears proven. However, a change in transformation logic alters key fields such as transaction type or counterparty classification. The dataset is complete, but no longer correct.
Delayed data breaking behaviour logic
Data arrives in full, but outside expected time windows. Monitoring rules relying on sequencing or behavioural patterns begin to produce inconsistent results. Timeliness is part of integrity, not an operational detail.
Fragmented ownership, unclear root cause
Source systems, data platforms and downstream tools each appear to function correctly within their own scope. The issue sits in the handover between them. When no one owns the journey, no one owns the failure.

Stronger organisations do not need more generic data-quality commentary. They need a sharper operating model for integrity: what to control, where to control it, how to evidence it, and who acts when it breaks.
